To Our Clients: Bring Proof of South Milwaukee Residency
To access SMHC, clients need to verify current proof of South Milwaukee residence. This is accomplished through a personal piece of mail, check stub, rent receipt, or official correspondence that indicates current South Milwaukee address that is dated within the 30 days of the visit to SHMC. Clients need to present this “proof of residency” on every visit. Clients can access SMHC one time per month.
